This Privacy Notice is designed to help you understand what kind of information I collect from you and how I will process and use this information.  

You have a right to know what data is held about you. This privacy notice will tell you what I do with any information I hold and how long I keep it for.

I (Sam Smith) am the data controller. I am responsible for collecting and processing your personal information. Processing includes the organisation, retrieval, consultation, use and deletion or destruction of information and its disclosure to other agencies.

With your consent I will record some personal and sensitive information. The personal information I collect will be your name, date of birth, address and contact details. The sensitive information I collect will be your gender, and any answers you provide me with via the questionnaire I send to you.

I recognise that the information you provide may be sensitive. I will store it securely and control who has access to it. I will not share any information where I am not legally required to do so or without your consent.

Corrections and concerns

If you believe that information I hold about you is incorrect or out of date, or if you have concerns about how I am handling your personal information, please contact me and I will try to resolve those concerns. If you wish to have your personal information deleted, please let me know and I will take reasonable steps to delete it. Otherwise I will delete your data after 36 months. If I become aware of any ongoing concerns or problems concerning my privacy practices, I will take these issues seriously and work to address them.
