Privacy Notice
This Privacy Notice is designed to help you understand what kind of information I collect from you and how I will process and use this information.
You have a right to know what data is held about you. This privacy notice will tell you what I do with any information I hold and how long I keep it for.
Data controller
I (Sam Smith) am the data controller. I am responsible for collecting and processing your personal information. Processing includes the organisation, retrieval, consultation, use and deletion or destruction of information and its disclosure to other agencies.

Gaining your consent
With your consent I will record some personal and sensitive information. The personal information I collect will be your name, date of birth, address and contact details. The sensitive information I collect will be your gender, and any answers you provide me with via the questionnaire I send to you.
Confidentiality and Information Security
I will keep your information confidential and I will store your information securely. I will not share your details unless
my duty to protect a child, a vulnerable adult, yourself or the public requires me to
you tell me about any terrorism or money laundering
I am required to do so by any court or law
I recognise that the information you provide may be sensitive. I will store it securely and control who has access to it. I will not share any information where I am not legally required to do so or without your consent.

As part of my professional responsibility as an accredited therapist of the BACP I have a monthly confidential meeting with a clinical supervisor to reflect on my work. I may refer to my session with you, but any reference will be anonymised.
Corrections and concerns
If you believe that information I hold about you is incorrect or out of date, or if you have concerns about how I am handling your personal information, please contact me and I will try to resolve those concerns. If you wish to have your personal information deleted, please let me know and I will take reasonable steps to delete it. Otherwise I will delete your data after 36 months. If I become aware of any ongoing concerns or problems concerning my privacy practices, I will take these issues seriously and work to address them.
